At the Children’s Heart Center, we work with patients and families to ensure they have the best possible experience before, during and after treatment for a heart condition. Our nurse navigators and referral coordinators work with patients, families and referring physicians to help ensure excellent communication and smooth transitions to Children’s for diagnostic testing, treatment and follow-up care. We provide seamless access to providers across the hospital for a coordinated, team-based approach to your patient’s care, providing information and educational resources each step of the way.

Additionally, we help coordinate your patient’s care by:

  • Gathering medical records and imaging results from other health systems.
  • Helping families navigate the hospitals and escorting them as needed.
  • Assisting with the hospital intake process and clinic visits.
  • Coordinating multiple tests to optimize sedation and time
  • Referring to case management for arranging the use of durable medical equipment after a child or teen is released from the hospital.
  • Coordinating interpretation services.
  • Coordinating financial planning with insurance companies and assisting with financial assistance if needed.
  • Communicating with you and coordinating requests.
