Statement Donation Form

Use the statement donation form during kickoff only when an agent would like to make a recurring monthly gift to the Aflac Cancer and Blood Disorders Center.

Employee Giving Form

Agents should fill out the employee giving form to determine what the per-pay-period donations will be to the Aflac Cancer and Blood Disorders Center.

Miscellaneous Donation Form

Use the miscellaneous form if agents would like to make a donation over a certain number of months to pay for an auction item.

In-Kind Donation Form

Use the in-kind form when obtaining a donated item from a company, organization or individual to be auctioned or raffled. It serves as a tax receipt.

Credit Card Authorization Form

Use the credit card authorization form when an agent would like to pay for an auction or raffle item using his or her credit card.

Tribute Form

Use the tribute form if an agent elects to make a gift in honor of or in memory of an individual(s) to the Aflac Cancer and Blood Disorders Center.

One easy way for Aflac Childhood Cancer Ambassadors to raise funds is to create a personal Click for Kids fundraising webpage and invite friends and family to donate online.

Are you on Facebook? Great news: You don’t have to leave the website to make a difference. You can help the Aflac Cancer and Blood Disorders Center by using Facebook’s fundraiser tool. Consider asking your friends to donate in honor of someone you know, celebrate your birthday or fundraise just because. Any amount makes a difference.
By organizing your own fundraising event, you can enlist your friends, family and co-workers to support your cause. No matter the size of your fundraiser, you can make a difference.
